-First month bill will reflect 1 month current and 1 month advance subscription fees(Bill).
(For example, for BB Deal 1 Mbps, RM110 x 2 = RM220.)
For New Line include
-Deposit Rm75(Bill)
-Stamp duty Rm10(Bill)
-Rm50 for Installation(direct to installer)
Note:
1. Minimum subscription period for Streamyx is 12 months.
2. Proceeding on site for Application form to fill in include signature of applicant.
3. Pcopy of IC / passport(Both sides) for Home / Resident
4. For business permise include pcopy of relevant supporting documents
a.Form 9 - Certificate of Incorporation
b.Form 24 - Return of allotment of shares (who holding the shares of the co.)
c.Form 49 - Statement of Particulars (who is director, manager and co. secretary)
d.IC / passport(Both sides)
